We bore beams before concrete pour if possible, otherwise we use a core drill after and sleeve the pipe in a metal guard. The hole does not weaken the beam if it is away from high stress zones.
What sets the height of pipes under the ceiling?
The first constraint is the required waste fall, the second is finishing space above the pipe. We calculate fall from sink point to stack, then choose the lowest height that permits it.
