Are paint flakes in the water harmful?
A coating not made for drinking water should not be in contact with it at all. The tank should be emptied and relined with a proper product.
Can the new lining go over the old paint?
Only if the old layer passes an adhesion test. Most failing paint must come off, because a new lining is only as good as what it sticks to.
