Can the hot water line from the heater run at the same diameter as the cold supply?
Usually no, because the hot line loses heat on its way so we use a slightly smaller diameter to keep velocity up. However, if the distance from the heater is very long, we may use equal or larger diameter depending on the flow rate needed.
What do we do if the water heater is far from the upper bathroom?
